Trouble with dates on support forms

Hey Truman,

I created a new form for submitting announcements to the church. http://www.ehawaiikai.org/announcementform.html

The date fields don't seem to be working right. 
a) the popup date selector doesn't come up (using Firefox, usually works)
b) when entered manually the date fields appear blank in the confirmation email.

Hi Koa,

Thanks for the bug reports! I've fixed both issues. The date picker icon shows now as well as the dates after the form has been submitted.
